Full-Time LPN Care Transition Specialist

Monday through Friday, 8:30a – 4:30p, no major holidays, no weekends.

The primary responsibilities of this job include the coordination, facilitation, and smooth transition of care provided to Total Senior Care PACE program participants who present at the ER, are admitted to the hospital, are currently in short-term or skilled rehab care or reside in long-term care, while assuring communication with the PACE IDT. The Care Transition Specialist is an advocate for the participant and the PACE program to ensure optimal delivery of quality participant care, the least restrictive and safest environment, and providing education to care providers as the PACE liaison

MINIMUM REQUIREMENTS:

1. Minimum Licensed Practical Nursing (LPN) degree required; experience with case management and/or working with a multidisciplinary team in a healthcare setting.

2. Two (2) years of nursing experience in a hospital, acute care setting, nursing home or community-based setting; preferably in geriatric care.

3. One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population.

4. Knowledge of PACE regulations preferred.

5. Demonstrated organizational skills.

6. Effective oral and written communication abilities.

7. Availability by phone

Acceptable driver’s license and use of automobile during work hours with current insurance.

Job Type: Full-time

Pay: $35,000.00 – $38,500.00 per year
